1. WHAT INFORMATION WE COLLECT
A) OUR SERVICES
We may collect personally identifiable information about our clients and users, such as your full name, phone number, email address, mailing address, geographic area, or similar, when you contact us or request our services. Through the process of conducting our business, we may also collect and retain sensitive personal data pertaining to health status and history (such as medical events and medications, for example) in order to provide safe and accurate treatment plans, advice and support. All information obtained is subject to strict confidentiality regulations and is stored electronically on a local device.
Patient records are kept in accordance with the Privacy Act 1988 (Cth) as amended under the Privacy Amendment (Private Sector) Act 2000 and the various State Privacy Acts. The Act and amendments regulate the use and the handling of personal information in private sector organisations including health service providers. The provisions of the Act and amendments impose regulations on health service providers to ensure personal information is kept secure and private.
B) OUR WEBSITE
We may collect basic visitor and user data when you navigate our website, including IP address, browser and device type, geographic location, and information about previous visits. Personal and business details may be collected from those who engage our services or contact us, such as when filling in a form on our website, purchasing a product, filling in our appointment forms, sending an email, joining our membership programs or mailing lists, or contacting us using other channels of communications.
C) ACCESS AND RETENTION OF DATA
We retain client records including personal data for as long as needed to provide our services and to comply with our legal obligations. An individual who wishes to correct, amend or delete inaccurate personal data should email to email@example.com.
We use the Google Analytics Data Retention controls to set a time period of 26 months before user-level and event-level data stored by Google Analytics is automatically deleted from Analytic’s servers. More information is available at https://support.google.com/analytics/answer/7667196?hl=en.
After approval of your comment, your profile picture is visible to the public in the context of your comment. If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognise and approve any follow-up comments automatically instead of holding them in a moderation queue.
We use the WordPress plugin WPForms for our contact and support forms on this website. When you submit a form your IP is recorded along with the information you provide, which is stored on our website’s database and transmitted to Nutrition for Ostomates. The form submissions are deleted from our website database at set intervals, however the information may be recorded on our email servers, and the third party services we use, for the purposes of managing our customer support and our business services.
2. HOW WE USE YOUR INFORMATION
We use the information we collect to operate and maintain our website, send you marketing communications, and to provide our services. Client records including personal data are used to provide the services and information that you request, to respond to comments and questions, and to provide support and service to our clients.
If you leave a comment on our site you may opt-in to saving your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you leave another comment. These cookies will last for one year.
3. HOW WE SHARE YOUR INFORMATION
Any personally identifiable information you make public on our sites through membership public profile pages or posting comments on our blog will be available to others. If you remove information that you have made public, copies may remain viewable in cached and archived pages of our website, backups, or if other users have copied or saved that information.
We work with partner and third party providers who provide services to us including processing transactions, email marketing, website development, website hosting, CRM and cloud hosting providers. These third party providers may have access to, or process client records including personal data, as part of providing those services to us. We limit the information provided to those providers to that which is reasonably necessary and take reasonable steps to ensure that organisations are bound by confidentiality and privacy obligations in regard to protection of your personally identifiable information.
We will disclose client records and personal data if required to comply with the law, to enforce our terms of service, or to protect the rights, property or personal safety of Margaret Allan, our users or others.
4. STORAGE AND PROCESSING
Your information collected through our services may be stored and processed in Australia, Singapore, the United States, Europe or any other country in which Nutrition for Ostomates or its service providers maintain facilities. We may transfer information that we collect about you, including personal data, to third parties across borders and from your country to other countries around the world, for the purposes as explained above in What Information we collect, How we use your information, and How we share your information.
5. HOW WE PROTECT YOUR INFORMATION
Margaret Allan is concerned with protecting your privacy and data, however we cannot ensure or warrant the security of any information you transmit to Margaret Allan, or guarantee that your information stored on Margaret Allan’s website and third party services may not be accessed, disclosed, altered or destroyed by breach of any of our industry safeguards. We follow generally accepted industry standards to protect the information submitted to us, both during transmission and once we receive it. When you enter sensitive information on our forms we use secure socket technology (SSL), however no method of electronic transmission or electronic storage is 100% secure. 6. PERSONAL DATA ACCESS, CORRECTION, DELETION AND OPT OUT
We respect your privacy rights and will provide you with reasonable access to the personal data that you have provided through our website and email marketing service.
If you have left comments on our website, you can request an exported file of the personal data we hold about you, including the data you have provided us. You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal or security purposes.
